Biguanide drugs related to PS-15 and its active triazine metabolite, WR99210, are in pre-clinical development for malaria treatment. The triazines of this series are inhibitors of the key plasmodium enzyme, dihydrofolate reductase DHFR ; . The WR99210 parent compound is effective even against the highly pyrimethamine-resistant quadruple mutant of Plasmodium falciparum 51I 59R 108N ; common in Southeast Asia. For many years, it has been assumed that antifolates are not effective against P. vivax, but recent work has demonstrated that pyrimethamine-sulfadoxine can clear many P. vivax infections. Even more encouraging, WR99210 is highly effective against a diverse set of dhfr alleles of P. vivax that have been identified in Southeast Asia. We have used a yeast expression system to screen compounds of this series for their effectiveness against DHFR of both P. falciparum and P. vivax. The most pyrimethamine-resistant alleles of each species, the 51I 59R 108N allele of P. falciparum dhfr and the 57L 58R 61M allele of P. vivax, were used as references in the screen. The results of the screen showed that the 4-tricholormethoxy- 2- chloro analogue of WR99210 is as effective in yeast system as the parent compound, WR99210. This compound [JPC-2056: 1- 3- ; propyloxy ; -5isopropylbiguanide] has now advanced to preclinical evaluation for development for treatment of both P. falciparum and P. vivax.

Because of the potential for serious adverse reactions in nursing infants from KEPPRA, a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother. 8.4 Pediatric Use Safety and effectiveness of KEPPRA injection in patients below the age of 16 years have not been established. 8.5 Geriatric Use Of the total number of subjects in clinical studies of levetiracetam, 347 were 65 and over. No overall differences in safety were observed between these subjects and younger subjects. There were insufficient numbers of elderly subjects in controlled trials of epilepsy to adequately assess the effectiveness of KEPPRA in these patients. A study in 16 elderly subjects age 61-88 years ; with oral administration of single dose and multiple twice-daily doses for 10 days showed no pharmacokinetic differences related to age alone. Levetiracetam is known to be substantially excreted by the kidney, and the risk of adverse reactions to this drug may be greater in patients with impaired renal function. Because elderly patients are more likely to have decreased renal function, care should be taken in dose selection, and it may be useful to monitor renal function and levonorgestrel.

Male aged 40 years with a body mass index BMI ; of 27.5. The subject began complete fecal and urine collection 24 h in advance of the 14C-B12 dose and continued complete 24-h collections until day 7. On the day of dose administration, the subject was fitted with an i.v. catheter in a forearm vein. Blood was drawn into 7-ml tubes containing EDTA. 1. Introduction Levetiracetam Fig. 1 ; is a novel antiepileptic drug which is structurally and mechanistically dissimilar to other antiepileptic drugs [1]. Its pharmacokinetic profile is linear with respect to dosage, its bioavailability is close to 100%, it undergoes only insignificant hepatic metabolism to inactive metabolites, it does not induce hepatic enzymes and about 91% of the dose is excreted via the renal route [2]. Therefore, it is close to a drug with ideal pharmacokinetic properties. Six treatments were compared in experi ment 1 : Glycine only; diammonium citrate only; glycine plus diammonium citrate; glycine plus glutamic acid; glutamic acid plus diammonium citrate; glycine plus glu tamic acid and diammonium citrate. In experiment 2, the standard mixture con taining glycine, glutamic acid and diam monium citrate was compared with glycine alone and with a mixture in which glycine supplied twice as much nitrogen as glu tamic acid. Each component of a mixture provided the same quantity of nitrogen except that in one mixture exp. 2 ; gly cine supplied twice as much as glutamic acid.
